MEDICAL ANIMATION TRANSCRIPT: El cirujano puede realizar una funduplicacin para tratar una enfermedad por reflujo gastroesofgico o para reparar una hernia hiatal. La digestin comienza en la boca. Cuando los dientes trituran los alimentos en pequeos trozos la saliva libera enzimas digestivas. Cuando se ingieren alimentos, estos ingresan al esfago, un tubo muscular largo que traslada los alimentos de la boca al estmago para continuar la digestin. Las contracciones denominadas peristalsis empujan los alimentos hacia el esfago. En la parte inferior del esfago, los alimentos atraviesan la vlvula muscular llamada Esfnter Esofgico Inferior o LES e ingresan al estmago. Los jugos digestivos segregados por el estmago son elevadamente cidos. Cuando el estmago se contrae para pasar los alimentos al intestino el LES se cierra estrechamente a fin de evitar que esos jugos cidos regresen al esfago donde pueden causar dao. Un msculo respiratorio llamado diafragma separa el pecho del abdomen para llegar al estmago, el esfago atraviesa el diafragma en un punto llamado anillo hiatal. Si padece la enfermedad por reflujo gastroesofgico o GERD, el LES no se cierra adecuadamente, dejando que los contenidos cidos del estmago vuelvan al esfago. Si la GERD no es tratada, puede producir diversos problemas esofgicos, entre ellos lceras, sangrados, estenosis o estrechamientos, y el esfago de Barret, que es crecimiento anormal de clulas tipo intestinal en el esfago que pueden producir cncer. Si usted tiene una hernia hiatal, una parte del estmago est sobresaliendo hacia la cavidad del pecho a travs de la abertura del diafragma. Una hernia hiatal puede afectar el LES e incrementar el riesgo y la gravedad de la GERD. Antes de iniciar el procedimiento, se le introducir una va intravenosa en el brazo para proporcionarle lquidos y medicamentos. Es posible que se le coloque un catter en la vejiga para drenar la orina. Se le administrar anestesia general que lo dormir durante todo el procedimiento. Una vez dormido, se le insertar un tubo respiratorio por la boca y en la trquea para ayudarlo a respirar durante la operacin. Para una funduplicacin, se puede utilizar uno de los dos mtodos. En el procedimiento abierto tradicional, el cirujano comenzar haciendo una incisin de ocho pulgadas, 20 centmetros en el abdomen para dejar a la vista el estmago y el esfago inferior. Despus de separarlo de las estructuras cercanas el cirujano envolver el fundus o parte superior del estmago alrededor del esfago inferior y lo suturar. Esta envoltura acta como un cinturn que ajusta y refuerza el LES. En el procedimiento laparoscpico, el cirujano comenzar haciendo cinco pequeas incisiones clave de aproximadamente media pulgada, 1,3 centmetros de largo en el abdomen. A travs de una incisin, el cirujano insertar el laparoscopio, un tubo largo y delgado que contiene una linterna y una cmara. La cmara enva imgenes a una pantalla de video para que el cirujano las vea. Se bombear gas para inflar el abdomen a fin de que el cirujano vea los rganos internos con mayor claridad. A travs de otras incisiones, el cirujano usar instrumentos para separar el fundus del estmago de los alrededores, envolverlo alrededor del esfago inferior y suturarlo. Si usted tiene una hernia hiatal, el cirujano empujar la parte herniada del estmago hacia el abdomen y ajustar el anillo hiatal en el diafragma para evitar otra hernia. Se puede realizar a travs de un procedimiento abierto o laparoscpico. Para finalizar la ciruga, las incisiones abdominales se cierran con puntos o grapas. Cuando se despierte de la ciruga, encontrar un pequeo tubo que ir desde la nariz hacia el interior del estmago.
